My Party agrees with Littlemonk - both as the rules state it, but also as its a quite powerful skill as it is.
However, how about the 6th possibility:
The WD has used his twin swords to continue his deathblow, but a 3rd monster is adjacent to him (but not next to the last monster killed), but he cannot continue his deathblow as he has already used the "sword change".
May he then move to another monster with dance of death? If so, does he need to break from pinning? May he move in contact with said 3rd monster (which he already is adjacent to)?

I think you are correct
kimjantzen, although there may be some confusion between Dance of Death skill and the twin swords - which are actually 2 different things and abilities.
I'm not sure if he should only be allowed 1 sword change per attack or if it should be per deathblow? It says "blocked square", not "squares". So I would think he can only skip 1 square this way, but if he can hit and kill his target I would allow him to switch blades again for the resulting deathblow.

Making several "sword shifts" would indeed solve the combination issue with dance of death, as you would be sure to reach one of the stopping criteria listed in DOD.
However, at least on my Twin Swords of Orion Card (yes I am one of those that actually play the WD ), the final sentence states: "...if the deathblow becomes blocked again, it stops".
In our party (our interpretation), the WD cannot use his DOD if there is any monsters adjacent to him - meaning that his attack would stop in the "6th possibilty" I describe above.
It doesnt make sense that he can use DOD to leap 6 squares through a corridor and continue his DB, but cannot do this if the monster is adjacent. However, as stated previously, both TSOO and DOD are both extremely powerful and nerfing them a bit only helps to balance him - not that the WD in my party needs nerfing; playing alongside a wizard and a lvl 7 pitfighter with 6 damage dices

Ah yes, you are right, it's pretty clear he can only swap blades once. (I swear I looked for that as I had read it recently)
I think the DoD deathblow is based on the WD's dancing style whirling momentum, I guess he loses rhythm if he encounters more than one obstacle.
